Introducing our Social Prescriber, Care Coordinator & Health & Wellbeing Coach

Aylesford Medical Centre are proud to introduce our enhanced roles within the surgery. 

Milly Netherton – Care Coordinator. Milly’s role within the surgery is to support frail and elderly patients who are needing multiple appointments and will benefit from help accessing community services, care homes and hospitals. 

Priya Chandan – Health & Wellbeing Coach. Priya’s role within the surgery is to support patients with low motivation and confidence who need help with their mindset and self management skills
