AMC Update on Review Invites

From April 2024 AMC are changing how we invite patients for their long term care reviews. This will include patients who have Asthma, COPD, Diabetes and Heart Failure.

Over the course of the next 24 months, the surgery will aim to invite and align patient reviews within their birth month. This will take time as some patients may have been reviewed within the last few months with their birthday falling before their next review due date.

Therefore we encourage patients to book in for their annual review once they have received their invite, in some cases this may also delay your review by a few months. As for all long term health conditions, if you have any concerns we would ask that you contact us as normal.
